Sunrise House is the only assisted living facility in Rhode Island licensed by the RI Department of Health and Department of Business Regulations for persons living with HIV/AIDS.

Sunrise House is designed to offer our clients a family atmosphere while providing essential care to encourage a healthy lifestyle. The Sunrise House program includes medical adherence, case management and social support in a drug and alcohol free environment.

Are you Eligible?

In order to apply for Sunrise House you must meet the following criteria:

• Written documentation of HIV/AIDS status
• Need for assistance with daily living skills
• Documented proof of a TB test within the last six months
• Engagement in HIV/AIDS specific case management
• Do not require any skilled nursing services
• Do not have persistent/prolonged mental health services

We offer specialized supportive housing for persons living with HIV/AIDS who are in danger of losing their permanent housing due to substance abuse. New Transitions provides a clean, sober environment to our clients.

By participating in New Transitions, our clients are able to:

• Address their substance abuse/use
• Develop life skills
• Receive counseling and assistance with daily living
• Maintain permanent housing in the community
• Create a stable environment for themselves
• Prepare for living a fulfilling life without substances

Are you eligible?

In order to be considered for admittance you must meet the following criteria:

• Written documentation of HIV/AIDS status
• Documentation of eviction due to substance abuse
• Commitment of at least a 45 day treatment period
• Must complete medical detoxification if necessary
• Must complete an initial activity plan
• Must commit to case management while involved in the program
• Must agree to transitional residency rules and adherence
