Title: Executive Director

Reports To: ACOS Board of Directors


Description: Executive Director is directly responsible for the organization’s consistent achievement of its mission and financial objectives.

This is a working manager position.  The individual will discover/develop new business opportunities, actively participate in the grant writing/submission process, oversee direct report programs such as housing, residential services and marketing/development, drive community partnership/networking, and be the public face of the agency.

The ideal candidate will have a background in social services and understand the challenges of a non-profit human services organization.

Responsibilities Include:

Leadership:
•         Assure that the Agency has and makes consistent and timely progress toward a long-range strategy in support of its mission.
•         Lead in the development and growth of programs, and organizational and financial plans.
•         Promote a healthy and effective work environment in support of the Agency policies and mission, and in compliance with all local, state and federal regulatory requirements.
•         Communicate regularly with the Board of Directors and senior managers to uphold and carry out agreed upon strategic and tactical plans.  Maintain a positive bi-directional flow of information with the Board and regularly provide executive reports to the Board and Executive Committee.
•         Act as the primary liaison with local, state and federal government agencies, staying well-informed of significant developments and trends, seizing suitable opportunities, managing risk, and advocating for the Agency’s mission.
•         Establish, maintain and cultivate sound relationships and cooperative arrangements with community groups and organizations, as well as with donors, foundations and other support resources. 
•         Be the public face of ACOS via community networking, public events, fundraising activities, and media publicity. 
Operations:
•         Approve the recruitment, selection, employment and release of all personnel, including paid staff, contractors and volunteers. 
•         Uphold a positive working climate which attracts, retains and motivates a diverse quality staff.
•         Discover and explore new business opportunities including programs, housing development and monetary funding streams.
•         Strategize the organization’s short and long-range development goals and fundraising plans.
•         Develop and maintain sound fiscal practices, including internal controls, budget preparation and operational adherence to budgetary and control guidelines.
•         Ensure sound and impartial human resources practices are in place.
•         Safeguard that all Agency practices are appropriate, legal and ethical.
•         Actively participate in the grant writing/submission process and audits for major grant funders.
•         Effectively lead all levels of Agency management, maintaining appropriate transparency of business information and objectives, while promoting a united mission.
•         Encourage and support staff development and education.

Job Responsibilities:
Cover lunch breaks and absences of full-time reception staff as needed.

Per diem Life Skills Specialist
 Brief Description:
•        Working with HIV+ substance abuse related disorders.
•        Assessment, intervention, facilitation of life skills and other residential duties.
•        Implementation of life skills curriculum for all residents.
•        Transportation of residents as needed.
•        Strong communication, organization, documentation and teamwork skills required.
•        BSW preferred, but equivalent work experience acceptable.
•        Must have transportation with valid insurance and driver’s license.
•        Bilingual (Spanish) a plus.

Seeking per diem CMTs for a 10-bed assisted living facility.  1st and 3rd shifts available and weekends.

Qualifications:
Must be certified in Rhode Island as a CMT and CNA.
Must be a mature, responsible, respectful and non-judgmental team player.
Must have reliable transportation and valid driver's license.
Bilingual is a plus.

Job responsibilities include:
Provide an awake, alert presence to monitor psychological well-being of residents, providing companionship and support as needed.
Foster a safe, hospitable, respectful environment that provides as sense of personal safety and well- being for all residents.
Administer medications, including associated documentation.
Provide transportation to residents as assigned.
Provide appropriate responses in times of crisis, conflict and medical emergencies.
Record accurate observations and data in each resident's file in appropriate format including activities, behaviors, vital statistics and other issues of concern in resident's life.

Title: Case Manager

Reports to: Case Management Supervisor

Position Type: 32 Hours/Week, Prorated Benefits

Brief Description:

The Case Manager is responsible for providing individualized service planning, advocacy, practical support, education, care plan coordination, and collaboration with other providers for persons living with HIV/AIDS.

Requirements:

Bachelor’s or Associates degree preferred. Two years relevant social work experience with diverse populations. Strong communications and organizational skills. Active driver’s license and insured vehicle required for taking clients to appointments as necessary. Bilingual (English/Spanish)strongly preferred. Direct service to homeless people, disabled people, people in recovery from addiction are preferred.

Housing Case Manager – Regular Full-Time with Benefits, $34,000 annually

Responsibilities:

·         Provides case management for HIV+ clients including,with a focus on comprehensive housing services:

o   Completion of intake and initial assessment;

o   Development and implementation of individualized Care Plan, reassessing every six months;

o   Meeting client face-to-face at least four times per year;

o   Contact with client a minimum of monthly;

o   Completion of release of information and updates;

o   HIV verification and nutritional updates at least every six months;

o   Enrollment of eligible clients in Medicaid, Medicare, ADAP, RI-FAB and other state/federal entitlement programs;

o   Facilitation of referrals for medical and support services.

·         Supports primary goal of actively maintaining client in medical care.

·         Provides intensive housing services including assessment, search, placement and related referrals.

·         Actively monitors clients’ medical conditions and plans for additional services to maintain an appropriate level of care as needs change.

·         Continuously monitors clients for changes in psychosocial needs.

·         Refers clients to in-house services such as Clinical Social Work.

·         Links clients to services provided by community based organizations such as peer support groups, legal services, clinical mental health services, nutritional counseling etc.

·         Educates clients on basics of HIV/AIDS infection,treatment, transmission, and risk reduction.

·         Advocates on behalf of clients, ensuring they have access to all services for which they are qualified.

·         Actively participates in in-service training, team meetings, and skill enhancement seminars.

·         Maintains timely and accurate client information in files and electronic CARE Ware system.

·         Ensures timely and accurate billing of services.

·         Transports clients as required to/from various appointments, while also enabling client’s independence and self-sufficiency.

·         Maintains client confidentiality and sets appropriate boundaries when interacting with clients.

QualificationsBachelor’s Degree or Associate’s Degree in human services or related field. Two years experience in human services.  Direct experience in a multicultural environment required.  Knowledge of HIV,substance abuse and RI housing resources strongly preferred.  Valid driver’s license, current insurance and reliable vehicle required. Bi-lingual (Spanish) preferred.

Peer Support Specialist – Part-Time, no benefits, $13.00/hour

Qualifications:

·        Share a connection to our client community; comfortable working with a diverse client community and dealing with sexuality, sex, addiction issues and other high risk behaviors.

·        Empathetic to clients living with HIV/AIDS and other infectious diseases.

·        Physically able to do community outreach without difficulty.

·        Professional manner, solid organizational skills, strong and culturally appropriate communications skills, high regard for personal and professional boundaries.

·        Reliable vehicle with valid insurance and driver’s license.

·        Familiarity with the current system of HIV/AIDS care services in the area is preferred.

·        Bilingual(Spanish) is preferred.

Roles & Responsibilities:

·        Partner with clients assigned by the Program Manager; help identify the individual’s barriers to remaining in care and maintaining medical adherence, etc.

·        Assist identified clients in navigating the system of care by accompanying them through the specific aspects of case management where assistance is needed.

·        Help identified clients become and remain fully connected to care, so clients can learn how to obtain and sustain needed support services.

·        Work with the case management/care team to identify medical and social services needed by the client, and help to provide information about available services,walking through key steps of the process that the client may find challenging.

·        Provide education and information about case management, ADAP, social services, as well as help clients identify needed documentation for determining support eligibility.

·        Help clients keep medical and other appointments and accompany them to medical visits and other appointments.

·        Coach and mentor clients by providing advice and emotional support; help the person living with HIV in making decisions to become fully connected to care.

·        Followup with clients to help ensure that a person living with HIV keeps appointments during the first 12 months of care, until they feel comfortable within the care system.

·        Provide health education about living with HIV and AIDS, prevention for positives, the importance of regular medical monitoring and medication adherence.

·        Offer partner services to persons living with HIV by connecting them to the health department’s partner services.

·        Participate in team and staff meetings.

Mental Health Worker – Regular Full-Time with Benefits, early-mid $40’s annually

Description:  Provide home-based outpatient mental health and/or substance abuse counseling, in the community, and/or a residential setting.   Therapy aims to help individuals affected by HIV/AIDS attain optimal functioning within their natural community environment.

 Responsibilities:

  • Provide home-based mental health and/or substance abuse services for people living with HIV and family members who have been affected.
  • Participate in case conferences within the programs at ACOS and provide support to case management team with clinical recommendations for client care plans.
  • Participate in a multi-disciplinary team to provide comprehensive and focus services.
  • Develop and co-facilitate treatment groups for adults/adolescents.
  • Conduct initial screenings, clinical assessments, and interventions with referred clients.
  • Maintain client records (progress notes, tx. Plans, billing)

Qualifications:  Masters Degree in Social Work or Counseling and relevant clinical experience working with individuals and families.  LiCSW preferred.  Demonstrate ability to work with diverse populations.  Ability to work within a multidisciplinary team approach.

AIDS Care Ocean State seeks an Adherence Nurse to work as part of our Coexist Housing Program.   This is a full-time position with benefits.

Responsibilities:

* Provide in-home medication adherence and nursing support to HIV+ clients.

* Work as part of a comprehensive team to support clients overall treatment adherence and health.

* Educate clients on their medical issues, medications, side effects, substance use, co-infection issues, etc.

* Work with physicians and pharmacies to ensure adherence to treatment plans.

* Maintain client files and documentation as required.

Qualifications: Active RN license in RI, experience with HIV, substance abuse and/or mental illness, ability to work with diverse populations and as part of a treatment team, valid driver's license with dependable vehicle/insurance, bi-lingual (English/Spanish) a plus.

Bingo Book Assembly

AIDS Care Ocean State hosts Drag Bingo from September to May each year. It is our largest monthly fundraiser, with over 500 people attending each bingo. We are in need of a large group of volunteers to assist our Bingo Manager in assembling the bingo books that are distributed each month. The time commitment is one day, to be determined between the group and our Bingo Manager. Week, week night, and weekends are available. Volunteers will assemble the bingo books at our office. We are looking for a minimum of 5 volunteers for this opportunity.

Monthly Bingo Volunteers

Interested in joining our team of dedicated Bingo Volunteers? Every month our Bingo Volunteers help out by setting up and decorating the Bingo Hall, recruiting teams of Bingo players, and selling Bingo Special Games during the evening. We seek energetic, dedicated and reliable individuals to join the team and all of the fun!

Our street outreach team goes out into the community and gives out safe sex kits, HIV testing information, toiletries, RIPTix, and referrals for additional resources to the at-risk population.
Street outreach provides the community with information on how to stay healthy, protect themselves, and protect others.

If you're not afraid to stand out, then become a Street Outreach volunteer! Our Outreach workers are easily identified by the bright yellow backpacks they each wear.

Safe Sex Kits

We distribute safe sex kits at health fairs, events, and at our Prevention Office. The kits contain safe sex materials and HIV testing information. Safe sex kits can be assembled at our office, at your home, school, business or organization. The time commitment is a minimum of one hour and can be arranged according to your schedule.

AIDS Care Ocean State is looking for HIV+ individuals who are interested in joining our Speaker’s Bureau. ACOS Speakers provide prevention and education information to a variety of groups in Rhode Island and Massachusetts by sharing their stories and speaking publicly about HIV/AIDS. AIDS Care Ocean State hosts numerous major fundraisers throughout the year that support our programs and services. We need volunteers for all stages of planning whether it is to sell tickets, help decorate, welcome guests, be a resource and help with all the little details that make our events amazing and exciting! 

ACOS prides itself as an organization that consistently develops and maintains successful and entertaining events. Volunteering at an event is a great way to have fun and support the work we do!  We welcome individuals and groups as well as one-time volunteers.
